SBNHS. The aim of the system was to establish an effective Intermediate Care Service, which offers a
co-ordinated interface between primary, secondary and institutional care and works collaboratively with voluntary agencies.
The service is available for adults who meet the referral criteria and who are registered with GP's currently within the Primary Care Groups in South Buckinghamshire.
